Located closer to where many community members live and work, the clinic offers essential services including Primary Care Medical, Dental, A&D, MAT, and Lab services.

If you are interested in becoming a patient, please fill out a New Patient Registration packet, found on our Forms & Policies page. If you are already a patient at the Siletz Community Health Clinic, you do not have to re-register to receive services at Dan’ ch’aa-may-yvlh-srii-dvn. Patients of both our Siletz and Lincoln City clinic locations will need to fill out an Annual Update Form to keep their registrations current.
