Shelf Stable Meals Available to Siletz Tribal Members

The Siletz Tribal FDPIR Program has received a generous donation of shelf-stable meals to distribute to enrolled Siletz Tribal members.

If you are interested in receiving these meals, please fill out the request form below. Requests will be open from April 14th, 2025 to June 30th, 2025. Meals will be distributed on a first-come, first-served basis.

The Siletz Tribal FDPIR Program is compiling a list of Siletz Tribal members who are interested in receiving shelf-stable meals.

*if person filling out form is an enrolled Siletz Tribal member. Non-Siletz parent/guardians may fill out this form if members of the household are enrolled Siletz Tribal members.
List all Enrolled Siletz Tribal members living in the household.
Use the plus button on the right-hand side to add another Siletz Tribal Member. Only list people who live in the household and who are Siletz Tribal members.
