Ordering Eagle Parts & Feathers
You can order Eagle Parts & Feathers from the National Eagle Repository through the U.S. Fish and Wildlife Service (USFWS). The Enrollment Department already has this form available to complete. If you need the Certification of Enrollment (found on page 3) filled out, email EnrollmentDepartment@ctsi.nsn.us or call 541-444-8258 to request the completed form be mailed/emailed to you. The address you have on file with Enrollment is what will be used on the form and where the original will be mailed to.
The USFWS will not accept a fax or emailed copy of this form. You can either download a digital form to fill out on your device and upload as an attachment on their online application, or mail-in a printed and completed application. It is the responsibility of the applicant to mail their completed form to the USFWS in their region. Additional information can be found on the website linked below.

Form BIA – 4432 is for Verification for Indian Preference for Employment in the Bureau of Indian Affairs and Indian Health Service. The Enrollment Department already has this form available to complete. If you need this form completed contact EnrollmentDepartment@ctsi.nsn.us to have a PDF copy emailed to you and the original will then be mailed to you at the address on file.
