Pharmacy Updates

 Due to pharmacy staffing limitations, we will be closed for lunch from 12:30 PM to 1:30 PM on Mondays, Tuesdays, Thursdays and Fridays. We apologize for any inconvenience and appreciate your understanding.

Ways to Refill Your Prescription

There are multiple ways you can refill your prescription at the Siletz Health Clinic Pharmacy.

Medical Clinic & Pharmacy

We are a AAAHC-recognized facility, here to provide comprehensive family practice medical services to Siletz tribal members, other Federally recognized Native Americans, and community members.

Services

Outpatient Services: Family practice/internal medicine, pediatrics, wound care, alternative care (medical massage), and women’s health exams.

Outpatient Procedures: Skin biopsy, toenail removal, pap smears, subdermal implant contraceptive (Nexplanon®), steroid injections, joint injections, and trigger point injections.

Diabetes

Medication Assisted Therapy (MAT)

Diagnosis and Treatment of Acute or Chronic Health Conditions

Psychiatric Services

Well-Child Care

Family Planning (birth control)

All CDC Recommended Immunizations

Minor Surgical Procedures

Laboratory (No in-house Radiology services are currently available)

Alpha-Stim Electrotherapy for treatment of anxiety, depression, insomnia symptoms, and pain.

Patient Appointments (Non-Emergency)

Appointments are made within our regular days and hours, excluding tribal holidays. We also offer same-day appointments for acute issues for already established patients.

Emergencies

We provide 24/7 phone triage for established patients. A provider is available for phone consultation after regular clinic hours, weekends, and holidays. Call the after-hours care phone and your call will be directed to our after-hours answering service. The after-hours care phone number is 541-444-1030 or 800-648-0449.

Same-Day Access

To make same-day access appointments for already established patients, call the clinic. You will be given an appointment time for that day or the next day with an available provider.

MAT Program

The Medical Assisted Treatment program is to treat substance use disorder for registered patients.

Diabetes Program

Appointments are available with a Certified Diabetes Educator.

Pharmacy

Pharmacy hours are M, T, Th, & F from 8:00 AM-5:00 PM and Wednesday from 12:00 PM – 5:00 PM.

Immunizations and Labs

Information on immunizations and laboratory services.

COVID-19 Information

COVID-19 vaccinations are available to Siletz Tribal Members and Registered Siletz Clinic Patients only, ages 6 months and older. The last updated version of the COVID-19 vaccine was September 2024. It is recommended that adults aged 65 years and older receive a second dose of the 2024 – 2025 formulation of the COVID-19 vaccine at least 6 months after their last dose. If you have older adults in your care who were vaccinated in September or October of 2024, additional doses are coming due in March or April of 2025. If you are interested, please contact your healthcare provider or visit your local pharmacy for availability.

The Siletz Clinic has Covid-19 Home Test Kits available at the Siletz Clinic Pharmacy, other Clinic staff can also assist you in getting a Home Test Kit.

If you have questions about COVID, or would like to discuss symptoms you are having or concerns, please call the clinic at 541-444-1030 and ask to speak to Nurse Triage.

If You Test Positive for COVID-19

If you have COVID-19 you do not need to isolate for a set number of days. However, you can still get others sick. If you have COVID-19:

  • Stay home until you have not had a fever for 24 hours without using fever reducing medication and other COVID-19 symptoms are improving.
  • Avoid contact with high-risk individuals like people who live in congregate care facilities or people with immunocompromising conditions for 10 days.
  • Mask when you are around other people in the 10 days after you become sick or test positive.

If you have a severe case, are hospitalized or are severely immunocompromised please follow the recommendations above, avoid contact with high-risk people and mask for 10-20 days. Please contact your healthcare provider for advice on how long to isolate.
