Attention: Important Announcement for the Change in Soft Open for the new Pharmacy Drive Thru
The Siletz Community Health Clinic new Pharmacy Drive Thru will not be opening December 31 due to multiple unforeseen issues.
The Soft Open for the new Pharmacy Drive Thru is rescheduled to January 13, 2025 at 8:00AM. Please note the Soft Open will be for existing Siletz Community Health Clinic Pharmacy patients, the Hard Open (open to all established patients of the clinic) is scheduled for February 10, 2025.
Until the Soft Open of the new Pharmacy Drive thru on January 13, 2025, the Pharmacy will be responding to phone calls to navigate and respond to prescription medication fills, refills, refill emergencies and questions. Please call the Pharmacy at 541-444-9625 or clinic at 541-444-1030.

We are a AAAHC-recognized facility, here to provide comprehensive family practice medical services to Siletz tribal members, other Federally recognized Native Americans, and community members.
Services
⚕ Outpatient Services: Family practice/internal medicine, pediatrics, wound care, alternative care (medical massage), and women’s health exams.
⚕ Outpatient Procedures: Skin biopsy, toenail removal, pap smears, subdermal implant contraceptive (Nexplanon®), steroid injections, joint injections, and trigger point injections.

Appointments are made within our regular days and hours, excluding tribal holidays. We also offer same-day appointments for acute issues for already established patients.

Emergencies
We provide 24/7 phone triage for established patients. A provider is available for phone consultation after regular clinic hours, weekends, and holidays. Call the after-hours care phone and your call will be directed to our after-hours answering service. The after-hours care phone number is 541-444-1030 or 800-648-0449.
Same-Day Access
To make same-day access appointments for already established patients, call the clinic. You will be given an appointment time for that day or the next day with an available provider.

COVID-19 Information
Covid-19 Vaccinations are available to Siletz Tribal Members and Registered Siletz Clinic Patients only, ages 6 months and older. The last updated version of the Covid-19 vaccine was September 2023. If you are aged 65 or older and have received an updated Covid-19 vaccine after September 2023, you are eligible for another vaccine. If you are interested, please contact your healthcare provider, or visit your local pharmacy for availability.
The Siletz Clinic has Covid-19 Home Test Kits are available at the Siletz Clinic Pharmacy, other Clinic staff can also assist you in getting a Home Test Kit.

If You Test Positive for COVID-19
If you have COVID-19 you do not need to isolate for a set number of days. However, you can still get others sick. If you have COVID-19:
- Stay home until you have not had a fever for 24 hours without using fever reducing medication and other COVID-19 symptoms are improving.
- Avoid contact with high-risk individuals like people who live in congregate care facilities or people with immunocompromising conditions for 10 days.
- Mask when you are around other people in the 10 days after you become sick or test positive.
If you have a severe case, are hospitalized or are severely immunocompromised please follow the recommendations above, avoid contact with high-risk people and mask for 10-20 days. Please contact your healthcare provider for advice on how long to isolate.
