Confederated Tribes of Siletz Indians (CTSI), Indian Health Services (IHS), and Siletz Community came together with the vision of a Siletz Community Health Clinic (SCHC).

The first SCHC building opened on February 9, 1991 on Swan Avenue downtown Siletz. The building accommodated Medical, Laboratory, Radiology, Pharmacy, Dental, Optometry, Community Health Advocate, Nutrition Education, Tobacco Prevention Education Program Coordinator, Mental Health Care, QI Coordinator, and Patient Accounts. Due to lack of office space Contract Health Services was located at CTSI’s Administration building and Alcohol and Drug Program was located on Old River RD. During our time in this facility, we added the Tillicum Fitness Center located on Buford Avenue.

Our new SCHC building was complete in April 2010. Since then we relocated our Contract Health Services (CHS) and Alcohol and Drug Program into our new building. Our programs have expanded services that include the CARE Program, Diabetes Program, Healthy Traditions, Early Intervention Services & Outreach, Tribal Non-Emergency Medical Transportation, A&D Peer Recovery Mentor, TLC Coordinators, Patient Benefits, Insurance Verification, and MAT Program.

Since then SCHC opened serving other community members, implemented the MAT Program, and opened Women’s and Men’s Transitional Living Centers.
