The Siletz Community Health Clinic strives to provide the best patient care to our beneficiaries. It is our goal to do so while adhering to the strictest standards of cleanliness regardless of the kind of clinic visit you may have with us. This survey is designed to gauge how our front-line staff performs with respect to hand hygiene, which is important in the prevention of the spread of infection.
